She was predeceased by: her parents, Roy J. Forsha and Daisy F. Brubaker (Berenbrok); her sister Karian Vaicka; her son Thomas L. Bernini; her sister Doris; her brother-in-law Donald Henigin; and her brothers, Harry Forsha (Huck) and Frank.
She is survived by: her daughter Debbie Zampino (Anthony) of Newton Falls; her brother Chris Forsha (Cindy) of Blairsville, PA; her sister-in-law Ann Forsha of Chambersburg, PA; her granddaughter Melissa; and her great grandchildren, Connor and Maggie. She is also survived by nieces, nephews & life long cherished friends.
The family asks that contributions go in the form of donations to St. Jude Children's Research Hospital 501 St. Jude Place Memphis, TN 38105.
