He was predeceased by: his parents, Louis Perkovich and Mary Perkovich (Shokac); his wife Beulah Smith; and his siblings, John, Tom, Louis, Kathryn, Margaret and Anne.
He is survived by: his son Larry Perkovich (Denise) of Newton Falls; his siblings, Rudy Perkovich of Warren, OH and Janet Wileman (Richard); his grandchildren, Victoria Coats, Andrea Perkovich, Joseph Perkovich, Phillip McHenry, Leigh Ann Smith and Eric McHenry; and his great grandchildren, Austin Coats, Waylon Coats, Hudson Gurmin, Khloe Perkovich, Samantha Perkovich, Olivia Smith, Gavin Smith, Claire McHenry and Rose McHenry.
