He was predeceased by: his grandson Cody; his parents, Richard and Debbie; his brothers, Dirk and Keith; his stepfather Clayton Harrell; and his former wife Haven.

He is survived by: his wife Janet; his stepchildren, Lisa Simms (Scott) of Parkman and Rocky Artino (Shani) of Macedonia; and his grandchildren, Kyle, Analeise, Ty and Olivia.

Visitation was held on Sunday, April 13th 2025 from 3:00 PM to 7:00 PM at the Sly Family Funeral Home (15670 W High St, Middlefield, OH 44062). A funeral service was held on Monday, April 14th 2025 at 11:00 AM at the same location.
