She was predeceased by: her parents, James Howard and Daisy Irene Taylor (Wallick); her husband Terry L. Polka; her brothers, Howard Taylor and Terry Taylor; her parents-in-law, Carl and Ruth Polka; and her siblings-in-law, Carl and Linda Polka.
She is survived by: her sons, Jason Polka (Trisha) of Dover, Jeff Polka of New Philadelphia and Todd Polka of Dover; her grandchildren, Chelsey Pestello (Evan), Chad Polka, Mitchell Polka (Taylor), Makenzie Polka and Kaylee Polka; her brother Bruce Taylor (Sharon); her siblings-in-law, Ted Polka (Brenda) and Patty Taylor; and her friend Dave Donato.
