He was predeceased by: his parents, Albert Hawk and Christina Hawk (Ott); his children, Jennifer Hawk and Rex Hawk; his grandsons, Daniel Erb and Joseph Hawk; and his siblings, Doris Harstine and Russell Hawk.
He is survived by: his wife Phyllis Hawk (Hahn); his children, James E. Hawk (Orlane) of Sherrodsville, Kimberly J. Erb (Greg) of Columbus and Timothy C. Hawk (Kristin) of Columbus; his grandchildren, James C. Hawk (Courtney), Michelle Zimmerman (Paul), William Hawk (Monica), Jessica Pierson (Randy), Sarah Kimble (Chris), Samuel Hawk and Spencer Hawk; and his siblings, William Hawk of Dover, Nancy Chovan of Massillon, Gordon Hawk of East Sparta and Robert Hawk of West Lafayette. He is also survived by numerous nieces, nephews and cousins.
