He was predeceased by: his parents, James Willard and Sara Jane Cook (Moore); and his sister Roberta Jane Martin. He is survived by: his sister Gay L Smith (Marc Warner); his children, Daniel Palmatier, Shawn Minard (Rob), Shannon Cook and William Cook (Cymoril); his grandchildren, Drew, Reece Palmatier, Dylann, Daxx Mullen, Hallie and Hannah Minard; and his nieces and nephews, Christie Kalikas (Greg), Adriane Williams (Jerry), Robert Smith Jr. (Christine) and Miranda Smith (Jenn).
