He was predeceased by: his parents, Ralph Carmack and Frances Carmack (Heavlin). He is survived by: his wife Joanne; his children, Caryn Duckworth (Andy) of Louisville, OH and Tim Carmack (Lynne) of Raleigh, NC; his grandchildren, Caitlin Duckworth (Javier Lopez), Christian Duckworth (Claire), Joseph Duckworth (Alexandria Anastasopoulos), Dana Carmack and Kiersten Carmack; and his great grandchildren, George and Lola Duckworth.
