He was predeceased by: his mother Josephine Tkacik (Fabian) of New Middletown. He is survived by: his father Paul Tkacik; his friends, John and Paul; his wife Denise Godina; his children, Alexa and Nicholas; and his sisters, Paulette Babyak (Robert), Patty Hanna (Charles) and Janet Flaherty (Tim). He is also survived by many nieces and nephews.
