He was predeceased by : his father Murry Chalk. He is survived by : his mother Rose Chalk (Svajlenka) of New Knoxville; his children, Theresa Reilly (Nathan) of Cincinnati, David Chalk (Sarah) of New Bremen, Michael Chalk (Melissa) of New Knoxville and Andrew Chalk (Lori) of New Knoxville; his siblings, Deb Kruse (Bob) of New Knoxville, Anne Hegemier (Tim) of New Knoxville, Rose Mary Wietholter (Brad) of Columbus, Carol Meyer (Preston) of New Knoxville, Lisa McCloskey (Tom) of Columbia and Kevin Chalk (Melanie) of Minster; and his grandchildren, Jonathan Reilly, Reese Reilly, Elise Reilly, Denise Reilly, Trenton Chalk, Ashlyn Chalk, Camryn Chalk, Ava Chalk and Logan Chalk.
