He was predeceased by: his parents, Paul W. Slack and Goldie E. Slack (Brown). He is survived by: his wife Loreita Slack (Cubbison); his sons, Jay Slack (Kim) and Troy Slack (Terri); his grandchildren, Grant, Tanner, Lyndi, Barton and Clayton; and his siblings, Paul Slack (Irene), Mary Jane Kirkbride (Bill) and Martha Jarrett.
