He is survived by: his wife Thelma; and his children, Jackie Ann Allen (Greg) of Lima, OH, Jeffrey Green of Santa Ana, CA, Michael Greene (Diane) of Pickerington, Janna Baugh of Pickerington, Jill Kennedy of Blacklick and Craig Matson (Lynn) of New Albany. He is also survived by 13 grandchildren and 7 great-grandchildren.
Interment Forest Lawn Memorial Gardens, In lieu of flowers, contributions may be made to Kobacker House in Jack's memory.
