He was predeceased by: his parents, Jack Lloyd and Mary Lucille Monk (Warren); his son Andy Wayne Monk; and his siblings, Judy Montgomery, Becky Monk and Terry Monk.
He is survived by: his children, Amy Jo Saving (Jeffrey) of Logan, Angela Jane Monk of Nelsonville and Allen Keith Monk (Laura) of Nelsonville; his grandchildren, Alecia Combs (Jarrod) of Lancaster, Alania Harper (Michael) of Nelsonville, Beth Primmer of Columbus, Heather Kerns (David) of Waynesville, Mallory Saving of Lancaster, Lacie Kaaz (Matt Bender) of Canal Winchester and Adam Kaaz (Shanda) of Nelsonville; and his sister-in-law Becky Figgins. He is also survived by 20 great grandchildren.
Donations in memory of Jack may be made to a charity of the donor's choice.
