He was predeceased by: his parents, David H., Jr. and Sarah Koch (Ferrell); and his father-in-law John "Vern" Lienhard. He is survived by: his brother Rick; his wife Joni Koch (Lienhard); his children, Scott Midcap (Jessi), Ryan Koch-Cate (Jon), Marcia Koch, Laura Koch and Sammi Jo Midcap Koch; his grandchildren, Nathan Midcap, Bronson Lee, Jermaine Pegram Jr., Stanley Pegram and Rozlyn Pegram; his brothers, John, David C. Koch, Robert and Pete Koch (Sheila); and his mother-in-law Nancy Lienhard. He is also survived by numerous nieces and nephews, and many friends.
