He was predeceased by: his great-grandparents, Richard Leroy Sullivan and Bertha Dyer Sullivan; his great-great-grandparents, Charles Shumaker and Geneva Shumaker; and his uncle Kevin Sullivan.
He is survived by: his parents, Cody L. Sullivan and Cynthia L. Caudill; his siblings, Jace Leroy Sullivan and Amelia Sullivan; his grandparents, Angela Dyer (Robert Evans) and Rodney Sullivan (Kimberly Bradley); his aunt Kayla Sullivan; and his great-aunt Charlotte Louise Sullivan.
Contributions may be directed to the funeral home to help the family with expenses.
