He is survived by: his wife Juanita Fay; his children, Matthew Fay (Wendy), Eileen Carr (Rusty), Thomas Fay (Katherine), Andrew Fay, James Fay (Lori) and Eric Fay; and his siblings, Doug Fay (Sharon) and Betty Lou Thompson (Jeff).
