He is survived by: his parents, Shane Polachek and Rhonda Bischoff (Fruth); his siblings, Zander, Gunner Polachek and Charlotte Polachek; his stepbrothers, Luke, Will and Colt Bischoff; his grandparents, Jerry, Sharon Polachek of Willard, Mike, Cecilia Fruth of Republic, Bradley and Linda Bischoff; his great-grandparent Jane Sharpless of Willard; his aunt Dawn Polachek of Willard; and his uncles, Adam Fruth (Sarah) of Republic, Todd Bischoff (Stephanie) and Jason Bischoff (Heather). He is also survived by numerous cousins, extended family members and friends.
