He is survived by: his wife Sue of Minerva; his children, Casey Thacker (Gary) of Peru, IN and Forest Edward Van Horn II (Marie Skiles) of Peru, IN; his stepdaughters, Denise Kennedy of Destin, FL, Kimberly Markopoulos (Robert Lewis) of Sagamore Hills and Melissa Stranger (Sean) of Canton; his grandchildren, Troy, Autumn, Olivia, Elijah, Nickolas, Noah, Rebekah, Rachel, Josh and Corey; and his siblings, Hubert Van Horn Jr. of Minerva and Velva Van Horn of Olive Hill, KY. He is also survived by nieces and nephews.
