He was predeceased by: his son Ty Everett Polen; and his sister-in-law Kelley Polen. He is survived by: his parents, Dave Polen and Marjorie Polen of Minerva; his wife Tracy of Minerva; his children, Ashley Lawson (Joe) of Minerva, Kalah Zwick (Mike) of Paris and Cody Polen of Minerva; his brothers, John Polen of Waynesburg and Tom Polen (Vivian) of Mineral City; and his grandchildren, Mikalyn Randal, Karah Randal, Ethan Randal, Tyler Zwick and Emberlynn Polen.
