Family and friends must say goodbye to their beloved Phyllis Ballard of Middletown, Ohio, born in Hamilton, Ohio, who passed away at the age of 96, on January 16, 2025. She was predeceased by: her parents, Frank Elmes and Emma Elmes (Buckley); her husband Herman Ballard; her grandson Joshua Steven Ballard; and her siblings, Emma Jean Rike, Mary Elizabeth Winterhalter and W. II (John D. Elmes).
She is survived by: her children, Steven Ballard (Jan), Diana Gerdau (Harry) and Jennifer Cox (Tom); and her grandchildren, Timothy, Phoenicia and Ethan. She is also survived by five great grandchildren; several nieces and nephews and many extended family.
Visitation will be held on Tuesday, January 21st 2025 from 12:00 PM to 1:00 PM at the Herr-Riggs Funeral Home (210 S Main St, Middletown, OH 45044). A funeral service will be held on Tuesday, January 21st 2025 at 1:00 PM at the same location.
In lieu of flowers, memorial contributions may be made to Hospice of Butler and Warren Counties, 5940 Long Meadow Dr., Franklin, Ohio 45005.
