He was predeceased by: his parents, Fred M. Cook and Garnet E. Cook (Wiley); his wife Debra B. Cook; and his siblings, Robert C. Cook, Fred M. Cook Jr., Mary E. Worrell and Janice F. Hartman.
He is survived by: his children, Charles M. Cook (Tracy) of Miamisburg, Joseph F. Boyd (Danielle) of Miamisburg, Bradley V. Boyd of Briery Branch, VA, Elizabeth J. Hines (Chad) of Miamisburg and Christy B. Stomoff (Stephen) of West Carrollton; his grandchildren, Kaetlyne (Nicholas), Brock, Bryce, Payton, Kayla, Paige and Parker; and his great grandchildren, Evan, Penelope Jo, Hendrix, Cherry Jo, Rhiannon and Beau.
