He was predeceased by: his parents, Donald Underwood and Louise Underwood (Newman); his wife India Marie Underwood (Moore); his children, Richard "Rick" Underwood Jr. and Cathy Shifflett; his son-in-law Thomas Folger; his grandchildren, Douglas, Amanda and Faith; his great grandchildren, Charles and Jayden; his great great grandchild Chloe; and his siblings, Donnie, Harry and Bonnie.
He is survived by: his children, Diana Cremeens (James), Cynthia Folger, Steven Underwood and Burnadette Underwood; and his brother Larry Underwood. He is also survived by numerous grandchildren, great grandchildren and great-great grandchildren.
