He is survived by: his wife Annette Heffelfinger; his daughters, Dena Adams (Cat) and Amber Perkins; his grandchildren, Makayla Baker, Aubrey Perkins and Bradley Perkins; his siblings, Tom Heffelfinger (Melinda), Robin Heffelfinger, Letha Mapes (Larry) and Jon Heffelfinger; his mother-in-law Theodora Chadeayne; and his siblings-in-law, Lon, Cher Lowery and Jen Hauff. He is also survived by many nieces and nephews.
In lieu of flowers memorial contributions may be made to Hospice of the Western Reserve Medina or the Alzheimer's Association.
