Stephens (Medina, Ohio), born in Whitley City, Kentucky, who passed away on October 6, 2019, at the age of 68, leaving to mourn family and friends. He was predeceased by: his parents, Theodore Stephens and Jalie Stephens; and his siblings, Scott (Wanda), Carl Stephens and Norma Jean Stephens.
He is survived by: his children, Leah Stephens and Adam Stephens (Kessiah); his granddaughter Freya; and his brothers, Jack (Juanita) and Robert Stephens (Cindy).
