She was predeceased by: her parents, Clyde Butcher and Ann Butcher; and her siblings, Marilyn Foreman and Michael David Butcher. She is survived by: her husband Richard Joseph Formani; and her sons, Richard Joseph Formani, Jr. (Shelley) and Jason A. Formani (Misty). She is also survived by her grandchildren and her great grandson, plus nieces, nephews and many friends.
A funeral Mass will be held on Saturday, April 26th 2025 at 10:00 AM at the St. Martin of Tours Catholic Church (1800 Station Rd, Valley City, OH 44280). A celebration of life will be held on Friday, May 9th 2025 at 6:00 PM at the Williams (787 Lafayette Rd, Medina, OH 44256).
