T. Hoffman; and his siblings, Marilyn Fouke (Jim), Jerry Hoffman (Luanne), Allen Hoffman (Bonnie), Ned Hoffman (Cynthia), Jim Hoffman, Joan Haugh, Diane Nordhoff, Denise Benschoter (Kevin) and Janet Wiley.
In lieu of flowers, memorial contributions may be made in Jack's name to St. Patrick of Heatherdowns and St. Jude's.
