He was predeceased by: his parents, Melvin Leroy and Kathryn Cook Sr. (Johnson); and his siblings, Charles and Nancy. He is survived by: his wife Katherine Ann "Kathy" Haines; his sons, Barte Cook of Delaware, Brett Cook of Marysville, David of Columbus and Eddie Cook of Columbus; his grandsons, NaRyan Cook and Brandon Stevenson; and his sister Marie Hendel (Tom) of Lake Placid, Florida. He is also survived by several nieces, nephews, cousins and dear friends.
