He was predeceased by: his parents, Helen Whitten and John Clyde Evans; and his sisters, Gina and Marian Louise. He is survived by: his son Assistant Medical Director; his wife Lorry Lentz Evans; his children, Laura Simmons (Joshua) of Delphos, John Evans, II (Donna) of Columbus, Jill Johnson of Marysville and Kelly Kramer of Marysville; his grandchildren, Jacob Evans Simmons, Kayla Bell (Michael), Alex Johnson, Kristin Johnson, Cameron Kramer, Carly Kramer, Samantha Leininger (David) and Jordan Simmons; and his great grandchildren, Saylor and Naomi.
Memorial contributions may be made to the Marysville High School Athletic Hall of Fame.
