She was predeceased by: her father James Edward Ford; her son James "Jimmy" Kozubal; her husband Edward "Phil" Kozubal; and her sister Barbara J. Levato (Ford). She is survived by: her son Justin Kozubal (April) of Marysville, Ohio; her stepchildren, Irene Buchholz (Joe) of Griffith, Christine Elmore (Terry) of Merrillville, Indiana and David Kozubal of Schererville, Indiana; her sister Charlene Kruse (Ford) of Hobart, Indiana; and her grandchildren, Erica (Wallace), Kara (Jon), Tony and Cameron. She is also survived by nieces, nephews, friends and extended family.
