He was predeceased by: his parents, Lewis E. Ballard and Mary M. Ingle Ballard; and his siblings, Forest "Red" Ballard and Linda Scottie. He is survived by: his wife Verna B. Andrews; his children, Suzanne Thompson (Dannie), Lewis Ballard (Rachel) and Jon Ballard; and his sisters, Betty Poling, May Parrot and JoAnn Owens. He is also survived by 9 grandchildren.
