He was predeceased by: his stepfather Jack Hollar; and his stepgrandfather Charles Blake. He is survived by: his wife Samantha DeGasperis Blackburn; his daughter Ava Nicole; his parents, Rhonda Hollar and Jasper "JB" Blackburn; his grandmother Laura Blake; his brother Shannon Blackburn; and his pets, Harley and Luna. He is also survived by many other family members and friends.
Memorial contributions may be made to Nationwide Children's Hospital "On Our Sleeves" a movement for childrens' mental health.
