He is survived by: his wife Leontine Van Dyke (Ray); his children, Ronda Hess (Ron), Michael Van Dyke (Angela) and Kevin Van Dyke; his grandchildren, Ryan Van Dyke, Erin D'Avanzo (Gill), Donna Laughman (Brent), Amanda Briggs (Beau Foreman), Keara Latham (Brett) and Kylee Van Dyke (Devon Wynkoop); and his children, Leora, Bella, Hayden, Kyla, Sean, Grayson, Landon, Barrett, Emmett and Easton.
In lieu of flowers, friends may send memorial contributions in honor of Jack to St. Jude's, the Shriner's or Tranquility Hospice of Delaware Ohio.
