She is survived by: her children, William Kincaid of Carrollton, Ohio, Karen Kincaid of Louisville and Daniel Kincaid (Diane) of Griffith, Indiana; her stepdaughters, Diana Smeltzer (Len) of Brookfield and Denise Martin-Buschan (Ken) of Tallmadge, Ohio; and her stepgrandchildren, Marie, Annie David, Gabriela, Richard and Donnie.
In lieu of flowers, the family requests donations that they will give to Hazel's favorite charities in her honor.
