She was predeceased by: her husband Gerald Haschak. She is survived by: her daughter Donna Smutak (Bob); her siblings, Gene Ulmer (Marilyn) and Dyan Kalal (Frank); and her grandchildren, Amber Sommer, Shawn Smutak (Tiffany) and Megan Jarzab (Josh). She is also survived by five great grandchildren.
