He is survived by: his wife Mary Beth Cook of Mt Orab; his children, Destinee Cook of Mt Orab, Jeremiah Carter of Lynchburg and Ryan Cook of Mt Orab; his mother Rachel Marie Smith (Fox); his brothers, Israel Cook, Steven Jordan (April) of Hillsboro, John Cook of Cleveland and Tim Cook (Amanda) of Lynchburg; and his cousin Andy Fox. He is also survived by many nieces, nephews, aunts, uncles, cousins, and friends.
