She is survived by: her husband Larry; her children, Tracy Jack of Willowick, Kenny Jack (Sarah) of Aurora, Tammy Mehaffie (Chad) of Louisville and Tommy Jack (Kate) of Youngstown; her grandchildren, Ayza, Ricky, Patience, Madison and MawMaw; and her siblings, Rick Crislip (Lisa), Craig Crislip (Karen) and Brenda Crislip (Junior). She is also survived by her in laws and many nieces and nephews.
