He was predeceased by: his parents, Martin Ortiz and Dominca "Maria" Ortiz; and his siblings, Lydia Munoz, Lucy Santiago, Jose Santiago, Juan "Cedo" Santiago and David Oritz.
He is survived by: his wife Deneen C. Frabotta-Ortiz; his children, Tiffany J. Ortiz (Jamie D. Burrer) of Elyria and Antonio W. Ortiz of Elyria; his grandchildren, Mckaelyn B. Ortiz and Savannah S. Burrer; and his siblings, Carlos "Rick" E. Santiago, Miriam Velez, Angel Ortiz, Caroline Ortiz, Mary Lou Ortiz, James Oritz and Jeanette Ortiz. He is also survived by many loving nieces and nephews.
