He was predeceased by: his wife Sabina Montanez. He is survived by: his significant other Edna Matos; and his children, Nitza Montanez (Manuel), Yvette Bell (Kim), Toni Acosta, Sylvester Montanez III (Lanisha), Moises Carreras Jr. and Juan Carlos Chamorro. He is also survived by grandchildren; twenty-seven great grandchildren; two sisters; two brothers and many nieces and nephews.
Because of his passion for and generosity towards St. Jude Children's Research Hospital, Silvestre's family suggests memorial donations may be made to St. Jude.
