He was predeceased by: his wife Elba Velázquez; his parents, Angelino Velázquez and Gertrudis Madera; and his siblings, Raúl Velázquez Sr., Florentino, Ramon, Euripides, Angel, Porfirio, Andy, José Velázquez and Maria Elis Velázquez (Anna Ramos).
He is survived by: his children, Milton Velázquez Jr., Nicky Velázquez, Jeannette Brown (Tony) and Windy Velázquez; and his siblings, Israel Velázquez, Heriberto Velázquez and Gladys Velázquez. He is also survived by 10 grandchildren; 18 great-grandchildren.
