He is survived by: his children, Sandra Cook (Kim) and Raymond; his grandchildren, Shelley (David) and David (Jessica); his great grandchildren, Ashlen, Baylen, Cullen and Riley; his daughter-in-law Nancy Kosakowski; his sisters, Theresa Tomaszewski and Helen Olexa; his daughter Kim; his granddaughter Miranda; and also Colleen Parker.
