He was predeceased by: his parents, Chester Chrulski and Jeanette Chrulski; and his brother Edward Chrulski. He is survived by: his wife Marlene Chrulski; his children, Tim Chrulski (Christine) and Mary Jo McQuaid; his grandchildren, Neil Vislosky (Chelsey), Gauge Settlemire (Megan), Joshua Chrulski, Tyler Chrulski, Tiffany McQuaid and Harleyann McQuaid; and his sister-in-law Jean Chrulski. He is also survived by several nieces and nephews.

Memorial donations may be made in Jack's memory to Nativity B.V.M. Parish.
