He was predeceased by: his mother Joanne Horak; his grandparents, Homer Blackstone and Margaret Blackstone; and his uncle Terry Blackstone. He is survived by: his wife Amy; his children, Damian Horak of Lorain, Jourdan Horak (Steve Kalapis) of Lorain, Joshua Horak of Lorain and James D. Horak of Lorain; his grandson Damian Horak, Jr.; his father Augustus "Gus" Horak of Sheffield Township; his sister Carolyn; and his nieces, Maggie and Ashley.
