She is survived by: her children, Walter Forman (Connie), William Forman (Denise) and Debra Ward; her grandchildren, Makenna Ward, Jeremy Forman (Tanya), Kelsey Santos (Raymond) and Keri Sanda (Milo); and her brother Charles N. Yost (Linda). She is also survived by many nieces and nephews.
