He was predeceased by: his father Leonardo D'Onofrio. He is survived by: his wife Diana; his daughters, Mea D'Onofrio and Grace D'Onofrio; his stepdaughter Amber Mitchell; his grandchildren, Lillianna, Brantley and Colette Mitchell; his mother Patricia D'Onofrio; his brothers, Leonardo Jr. D'Onofrio (Lori) and Joseph D'Onofrio (Corrine); his nieces and nephews, Crystal, Isabella D'Onofrio, Joseph Jr. and Domonic Leonardo D'Onofrio; and his great-nephew Logan Hill.

Per CDC guidelines, face coverings and proper social distancing will be required at all times.
