Forrest (Jessica) of Logan, Ohio and Matthew Michael Forrest (Cori) of Logan, Ohio; and his brothers, Richard Ray Forrest (Donna) of Lancaster and Randy Neil Forrest of Logan, Ohio.. He is also survived by several grandchildren; several great-grandchildren.
