He was predeceased by: his parents, Malcolm Paul and Alva Laverne Cook Sr. (Bailey); and his siblings, Paul, Aloma and Debbie.
He is survived by: his wife Judith Ann Cook; his children, Tracy L. Cook (Bobbi) of Rockbridge, Brad Cook (Cindy) of Logan, Ohio and Carrie Losey (Shane); his grandchildren, Cole Cook, Cailyn Cook, Randy Cook, Jenny Gladman, Cody Joy, Carlie Losey and Collin Losey; his great grandchild Kelsee Joy; and his siblings, Ben Cook of Lancaster, Barney Cook of Logan, Ohio, Jean Courtney of Logan and Eydie Wittekind of Logan, Ohio..
