He is survived by: his wife Debra Sue Cook; his children, Michelle Walsh, Kelly Milam (Terry), Courtney Williams (Elijah), Michael D. Cook Jr. (Paula), Ronald Osborn (Veronica Carbetta) and Joseph Cook; his grandchildren, Michael (Rebecca), Jonathon Avery, Amber Blackwell (Chris), Ross, Lorrie Pittman, Alex, Kaley, Alanna and Declan Williams; his great grandchildren, Nathaniel Pittman, Jackson Blackwell and Zelda Avery; and his siblings, Judith Flood, Linda Ferguson (Sonny), Susan Marteney and Phillip Cook.
