He was predeceased by: his parents, Benjamin Richard and JoAnn Marie Chambers (Malone); his granddaughter Ashiah Chambers; and his brothers, Richard and Gary Chambers.
He is survived by: his wife Paula Faye Chambers (McGlothlin); his sons, Jack Chambers Jr. of Logan, Mark McGlothlin (Shana) of Athens and Jordan Chambers of Logan; his siblings, Greg Allen Chambers (Rose) of Logan, Brenda McClelland of Logan and Lisa Cox (Rick) of Logan; and his pet Marlee. He is also survived by five grandchildren.
