He was predeceased by: his parents, John A. Sr. Gephart and Mary Gephart (DeMange). He is survived by: his spouse Starr Cook; his sons, Jeff Gephart (Kelly) of Ft. Loramie and Jack Gephart (Renee) of Ft. Loramie; his stepchildren, Andy Cook (Donna) of Brighton, MI and Nicole Tata of Lima; his grandchildren, Luke, Seth, Kennedi Gephart, Janelle Meyer (Chris), Darren Gephart (Taylor), Clara, Levi Gephart, Alexis McDermott (Hunter), Greg Cook, Jessica, Rose and Travis Tata; his great grandchildren, Amir, Kaden, Myla, Luca, Warren, Harrison, Allie and Benjamin; and his brother Jerry Gephart (Linda) of Newport.

In lieu of flowers the family would appreciate donations toward establishing a baseball scholarship at Fort Loramie High School for a player attending the University of Toledo.
