He was predeceased by: his parent Aljean Vorhes (McPheron). He is survived by: his father Ralph N. Forte (Judy) of Port Charlotte, FL; his children, Jason Hamlin of Wapakoneta and Jennifer Ballard of Dayton; his grandchildren, Jameson Hamlin and Darius Ballard; and his siblings, Allen Vorhes (Lisa) of Sandston, VA, Debra Roots (Darren) of Lima, Tomi Carmean (Rodd) of Hilliard, Nicki King (Erik) of London, OH and Tony Forte (Melissa) of Grove City. He is also survived by nieces, nephews and great-nieces and nephews.
