He was predeceased by: his wife Alice; and his brother Daniel Holly. He is survived by: his daughter Tami Slaise (William) of Lima, OH.; his grandchildren, Rashad (Sparkle), Holly, Skyler, William III, Thalia and Constancia; his sister Lynn Phillips (Porter) of Las Vegas; and his sons, Richard Parrish and Mark Cousey. He is also survived by several nieces, nephews, cousins, other relatives, and many special friends.
